Cremations

Funeral Directors will need to submit the following completed forms for a cremation

All submitted forms are required to be correctly completed and any discrepancies within them will be queried by our bereavement team. The cremation will not take place until these discrepancies are rectified.

The Hull City Council Bereavement Services Preliminary Form is available upon request. Please contact us for this.

The green or white form is provided by the Registration Service upon registering the death.

Funeral Directors can access other cremation guidance and associated documents they require on the Government Website.

Burials

Funeral Directors will need to submit the following completed forms for a burial -

Payment of Services

New Funeral Directors will initially pay for services upfront and on a pay as you go basis before services can proceed. Existing Funeral Directors will be invoiced for services carried out within the month on the last working day of the same month. If invoices are left unpaid, Funeral Directors will revert back to the new Funeral Directors terms as above.
